Southwell Health and Rehabilitation in Adel earns 5-star CMS rating

Southwell Health and Rehabilitation (SHWR) in Adel has earned a 5-star overall quality rating from the Centers for Medicare & Medicaid Services, the highest rating available for nursing homes.

The 5-star rating reflects a major improvement for the facility, which moved from a 3-star overall rating to a 5-star overall rating. CMS ratings are based on health inspections, staffing and quality measures.

Located on the Southwell Medical campus in Adel, Southwell Health and Rehabilitation provides skilled nursing care, short-term rehabilitation and long-term care services for patients and residents throughout the region. The facility serves individuals recovering from illness, injury or surgery, as well as those who need ongoing support in a residential care setting.

“This achievement is a direct reflection of the hard work, dedication and compassion of our team at Southwell Health and Rehabilitation,” said Christopher K. Southwell Health and Rehabilitation is one of 353 nursing homes in Georgia evaluated through the CMS Five-Star Quality Rating System. The system was created to help residents, families and caregivers compare nursing homes and make informed decisions about care.

The designation also highlights the facility’s continued focus on quality improvement, resident-centered care and support for families throughout the region. From therapy and rehabilitation services to daily nursing care, Southwell Health and Rehabilitation works to help residents maintain comfort, dignity and the highest possible quality of life.

Joined by residents, their families, SWHR staff and leadership, the facility recently celebrated their achievement with a program and reception. Remarks congratulating the team on the accomplishment were given by Mark Kimball, Chief Operating Officer for Southwell Medical; Chris Dorman, President and CEO of Southwell; Dr. Tom Fausett, Medical Director of SWHR, Chase Daughtrey, Southwell board member; and Pam Clayton, Vice President of Quality Advancement and Regulatory Affairs for the Georgia Health Care Association.
